How to Apply for a Visa

Visa Request Form DS-156 (pdf)

Supplementary Form (DS-157): All males between 16 and 45 years of age must fill out a supplementary form (DS-157) together with the non-immigrant visa request form (DS-156).

Note: applicants for diplomatic or official visas do not need to fill out this form.

Student Visa Form (DS-158): to be filled out by all F,J and M applicants together with the DS-156 and DS-157 (if appropiate).

New Requirements for F, J and M visas: Starting September 11, 2002 there are new requirements for F,J and M Student Visas. Your application cannot be processed until we receive confirmation from your institution, either in the ISEAS system online or via e-mail directly from your institution.
