BASIC REQUIREMENTS
Valid passport.
Complete form DS-156,
available at the Embassy reception desk (Monday –
Friday from 9AM to 5 PM) or your travel agency. All males
between 16 and 45 years of age must also fill out an additional
form (DS-157).
One recent photograph,
clear, front view, full face, plain light background,
black and white or color, 50mm by 50mm.
$3000 (Uruguayan pesos)
for each application. No checks and credit cards are accepted.
This amount should be paid in advance at Indumex and is
not reimbursable even if the visa is
refused.

SUPPLEMENTARY DOCUMENTS
Each visa applicant must
clearly show to the satisfaction of the Consular Officer
that he or she has compelling reasons and responsibilities
abroad which will ensure their departure from the United
States after a temporary visit. For this reason it is necessary
for each applicant to present documents showing ties to
his/her country of residence and financial situation.
